BC has a number of different programs. You can read about each of them and their requirements here: https://www.welcomebc.ca/Immigrate-to-B-C/About-The-BC-PNP

I'm not sure what you mean by "federal work visa". If you want a work permit, you need a job offer from an employer in Canada and that employer needs to obtain an approved LMIA so that you can apply for a closed work permit.

BC PNP paths require a job offer from employer who cannot find a Canadian/PR to do the job.

Research each PNP program. You cannot work in Canada as a doctor unless licensed. Need to research about IMEs in Canada. Nomination is based on age, work experience, language, education and funds. If over 40, it makes it more difficult to immigrate.
