AOR for Proof of Citizenship received - with the WRONG name on it?

My son was born outside of Canada, in the USA. His mother is a Canadian citizen and we live in Canada so we applied for a Proof of Citizenship for him. We just received the AOR and everything looks good except the AOR is addressed in my name, not my son's name. Three years ago we did the same process for my daughter who was born outside of Canada and the AOR came in her name. The application was sent in his name, not mine....1. why would the AOR be in my name? 2. do we need to do anything about it? 3. what do we do?
