Annex A with PPR

Hi Guys,
Need your help!!
I got passport request from delhi immigration for PNP PR. In the list of documents it says I need to submit
1.passports
2. Photos
3. Email copy
But in some of thread people said they got Annex A form to submit also. I checked my email (forwarded from my representative) but I didn’t find any annex a form. Is it normal? Not everyone has to send annex A?

Please advice!
