Amending Canadian Birth Certificate

My Canadian husband is trying to amend his birth certificate to have his deceased father's name included on it (his mom left it out when he was born) before we submit our family class sponsorship application forms. Office of the Registrar General sent him an "Application to Amend a Birth Registration to Add a Parent's Information" and it says "If one parent is incapable, a Form 33, Statutory Declaration by One Parent or Third Party, must be completed and attached." regarding his father being deceased and unable to sign. I was wondering if there was any way to get this form online or would we have to call the Office of the Registrar General and request it? Service Ontario does not have this form or know what it is.
