Affidavit

Hi all, As there is a difference in my name as per passport and Educational credentials, im supposed to provide an Affidavit while doing IQAS for ECA purpose. But im confued with respect to the format of Affiadvit. Is there any specific format for Affidavit ??? Im from India, here for legal validity Affidavit should be in a stamp paper.
So, should i write in normal paper and sign it and upload or should it be in a stamp paper??
