Affidavit Validity Period

Hello There,

My name is spelt differently in various documents. For this purpose I have an affidavit that was drafted around 5 months ago explaining that it is the same person with names spelt differently.

Will this affidavit be accepted now. Or should I get a new one.

Also is it enough is the affidavit is signed by a lawyer only or down it need to be attested by any other body?
