Affidavit that applicant has opted to use maiden name for Philippine Passport

Hello! Could anyone please help me regarding renewal of PHILIPPINE PASSPORT? I won't be using my husband's surname in the passport and I see in the consulate website that they require an "affidavit that applicant has opted to use maiden name", where do I get this document?

Wish I could help you but your question seems to have nothing to do with the topic of Canadian Citizenship..Perhaps try to ask somewhere else or call the Philippines Embassy/Consulate?
