Affedavit of spouse name changes

Has anybody created an affedavit for spouse name changes and submitted it to WES?

My Spouse last name in degree certificates is different than in Passport as we got it changed after marriage. Do we need to sumit an affedavit to WES. I am sighting that they might ask for it so can anybody help us with the format?

No they won’t ask- I got for my spouse.
