Advice needed for IMM 5409 Statutory declaration of common-law union

Hi all!

My partner and I currently filling up the IMM 5409 form and would need some help to fill in which country in the form (as there is only 1 space to fill up for the country). We are from different home countries Hong Kong and Malaysia but we have lived together from year 2014-2016 in the UK. As we are getting the form signed with the presence of Notary Public in Malaysia, should we fill in Malaysia as the country in the form?

have you managed to get an answer to your question? I am also confused about how to fill that form. should it be like this we A and B of cityA and cityB country of countryA and countryB in cityC in the country of countryC?
countryA and countryB are our nationalities.
countryC where we are living at the moment.
