2) I worked with off campus work permit when I have official 4 months off after two semesters from college so what should I mention that "Were you a full-time student during any of the above listed periods? YES/NO"

--- Write your actual address. I hope you have used your Home address at some stage in your life. What was you address when you were in school or see you birth certificate. (I have my home address mentioned in BC)

-- Did you graduated after completing 2 semesters ? OR Did you had 4 months break in college? Please be more specific to answer you question.

I assume this is the 'Contact Information' question on IMM 0008. The form will let you leave 'Street no.' blank, but not 'Street name'. If your current address really doesn't have a street name, I suppose you could put "N/A" in here?

If this was before you graduated, then you should say "yes." If it was after you graduated, you should say no.
