Hello,

I was wondering if someone can help me out. My husband received an e-mail indicating he has to do redo his medical exam, as they are going to expire soon. I am having trouble understanding what the File number B and EP mean. I know they do not mean the UCI or or the application number. I think the EP is the number the medical examiner will give to my husband once he does the exam again. I would greatly appreciate if someone can clarify this for me, especially the File Number B. Below is the information they ask for.

Thanks so much!

ADDITIONAL MEDICAL INFORMATION

File number: B__________________ EP_________________________

Look at his acknowledgement of receipt(AOR) letter or email - there will be a file number that starts with B. I don't know about the EP number.

Thank you for your response. Do you mean the e-mail, that they sent my husband indicating that that he has to redo his medical again. It says Ref B06.... But on the application form for additional medical information it indicates File Number B

No, I mean the letter he would have received with his FIRST medical request? That letter usually has the client ID #, a file number that starts with B and an application file number that starts with E. Anyway, if all fails, he can print the email request and take to the DMP - they can figure it out for you.
