Additional Family Information

Morning guys, i need some help filling out form IMM 5406 Additional Family INformation there are three diferent dates, should i put info there no matter if i am not signing because i am married and i have kids?
That part is kind of confusing, please advice.

spaces where you have to put a date, thre are three of them, i know that the last one is gonna be use when you sign the form, but what about the other two?

You dont have to put dates their if you are not signing it just leave it and out date and sign in the last one only.
As you hav wife and kids so dont have to sign and put dates in other two.
