ADDITIONAL FAMILY INFORMATION - Names in native language

Hi Friends,

I have this question about how to write your name in English AND your native language in the small scape provided for this in the box.

Solution could be:
1. I use this space to write the name in english with completing all other information on the form. PLUS additional same form with Name Written in native language with Pen with all other information completed on the form. So i will have two copies of this document one on name will be in english and other name will be in native language.

2. I complete the form with name in english and provide the native langaue name information on another paper with additional partial information associated with name in the boxes (tables)

I assume you native language is Hindi, right? Well.. both ways are correct. Several of members, included me... printed name in english on computer, then wrote with hand using blank ink on same form and boxed. its look like small, but you can easily write if you want to do

Then second way is correct too, you can fill form in English, then attach one extra sheet with all names in your native language on It. P.S, if you can draw one table for names as on orignal one, that would look like more perfect . Good luck

bad idea as per me. 2 copies of the same form can be confusing

can do, but why even put the name in the box in english? just write your first and middle name name in Hindi, then, put an (*) below the box and write in english "complete name in native language provided on attached sheet"
