AS per Guide Additional Family Information [IMM 5406] application has to be filled by applicant and all others included in the general application
My question:

If I file the application as a "APPLICANT" in section A of 5406 i have to include:
- my personal info in applicant row
-my husband info in SPOUSE row
- my mother info in MOTHER row
- My Father info in Father row

If my husband files the 5406 form what he has to write in all those rows?

(as I understand he has to say my name in APPLICANT, HIS info UNDER SPOUSE and his mother and father info)

Please HELP ME I am confused as we have had the same last names before and after marriage and the officer might be confused which application is my and which one is his

when you are filling up the form, you are the applicant, when your spouse is filling up the form, s/he should be the applicant. a little confusing, but many people did it this way in FSWP. this applicant has nothing to do with who is the principle applicant.

Second that.

I entered the information as suggested by OP and I had no issues all what so ever with my appliaction.
 
my father-in-law died in march 2016 so I want to know should I write my mother-in-law marital status widow and for my father-in-law?
