According to CIO checklist: Additional Fam Information Completed By
. The principal applicant
. Spouse
. Children above 18

Does this mean form has to be filled by me and a separate form by my wife and separate forms by any children above 18 years old?
 
Hi,

I think there is a confusion in my question and your answer. The Additional Family Information form IMM5406 (which has 1 page) needs to be filled only once. It contains information about principal applicant, Spouse and Dependent children. I don't think that each member of my family needs to fill in a seperate form for themselves.

I checked the CIO checklist and guide and no where does it mention that each member needs to fill a copy of this form.

Please correct me if I am wrong. Any expert advice would be appreciated.

this need to be filled by every member over 18 you will see there is an option in the form to select for spouse or child or applicant on the top.
 
Principal applicant "and " anyone over 18 included in application has to fill a seperate form.

@dipmm2000: sorry but I couldnt find any place on the IMM5406 form where we can indicate who is filling the form.

However, I see that most people are recommending to fill it separately so I will do that. And i dont see the harm in filling in an extra form from my spouse. Only question would I mark her as applicant in her form or will I remain the applicant?

yes , spouse will the applicant in that case

Dear Moby:
Just take as many print outs of this form as many members of your family including you, your spouse & children over the age of eighteen years.

e.g if you have three children over the age of eighteen, you need to fill 05 forms. Same form but filled with different info & signature for every family member. Hope u got the point.
