In form 5406 (Additional Family Info) - It says: Complete ALL names in English and in your native language
Should I write names in Hindi as well? I do not remember sending names in Hindi in the form submitted to BUffalo office before.

Please guide.

Yes, my wife wrote her name in both English and Chinese.

If your/her native language is Hindi ,yes you must have to write names in English as well as in Hindi on Form5406
 
Also if there is no space left on the form, Attach an extra sheet (make it in MS word), make a table 3 columns, Type in the Names in English, then date of births in the next column.
And then write with a pen (I used black) names in native language.
Then staple it at the back of the Family info sheet.
Also on the top where it says write the name in native lang. I wrote with hand, "see attached sheet for names in native lang."

I did it little different. I typed Hindi fonts, printed on sticky paper, cut in small pieces and pasted besides english names. Hope that's fine.

I hav one more question regarding this form.
In what manner will spouse fill this form? Whoz name will come in applicant and whoz in spouse?

In the spouse form the applicant name will be your spouse name.
 
Well I wont do that, I mean sticky paper thingies. It says on the form "Type or Print in black ink". Dont make it complicated ;D
