Adding twins to PR application

I need to add my new born children(twins) as dependent to my current PR application.I have been asked to upload IMM0008 and IMM5406e forms along with other docs.such as passport,birth certificate etc.Do i need to fill separate IMM5406e form for each child or add both of them as dependents in a single IMM5406e form?
I am really confused becoz i could see 2 upload placeholders in my Application with description as 'Kit,Application forms'.Is one placeholder upload option for each child?

Thanks.....also one doubt..For each child,do we need to merge the IMM5406E Form filled by me and spouse separately,IMM0008 form, birth certificate,passport and other docs as a single pdf and upload in the placeholder option provided?
