Academic standing requires me to discontinue

hi,
my academic standing requires me to discontinue and i've a doctor's note to back up my poor grades.as a result of this, i applied to a college which has accepted me.my study permit expires in a month's time and i'm concerned if i will be issued a renewal of my study permit. also, i was wondering if i should add a doctor's note to my renewal application. thank you
