Do I need to write N/A in the box? Or just leave it blank?

1. it clearly indicates that only if yes which is not the case so leave it unchecked.
2. Where exactly are you thinking of writing n/a?
3. Why are you referring to table B where it is clearly a part of 14.b and not the section you are asking about?

Sorry, "table B" is wrong. It should be "it".
