Hello, I know this question has been asked before. And I looked up many sites online, here is the following document I prepared for flagpoling.
(1) last three month bank statement and T4
(2) passport, COPR
(3) Two 50mmx70mm photo
(4) Job offer and reference letter
(5) previous work permit
(6) birth certificate
(7) medical report (6 more months before expiry)
Am I missing anything else that I missed here? Should I pre-print out the application form (some IMM forms maybe)? Any senior can shed some light on this? I am concerned that if I miss anything and I will have to travel all the way back from border and do it again, which is half a day by bus.
