My wife has changed her surname after marriage in all documents except the passport.

Now, while filling up the form, should I mention her maiden name or her married name ?

We can also get her name changed on the passport, but I guess, they issue a new booklet in case of name change and this takes time and it might delay the process of application

pls advice as to what should be done? is it ok to apply with maiden name and later change it to married name or the better thing would be to continue with her maiden name only?
 
You are fine,It is Canadian custom for many Canadian women to keep their Married names,If it is good enough for Canadian Politicians,Business leaders,its good enough for your wife,just show proof of marraige
