5 years old one and same name affidavit

I am applying for a Canadian Tourist visa. I have 'One and the same name affidavit' which I got prepared 5 years ago in 2016.
Can I use the same affidavit for this application or should I get a new one?

No, it will not work. You need to prepare a new one.
