If uh were approved and uh sent ur passport and nd they say no decision made. May be ur passport is going to expire/ pages full/ papers cracked/ passport seems damaged. They can return ur passport based on any of the above.
If you were not approved, applied online or offline and no decision made : that means your paperwork was not filled properly or something missing or some documents. Incomplete applications are returned.
