Yes, they have requested reason for refusal of US VISA that i had applied in 2014,

This is not applied by myself yes.

In that case it sounds like they feel you didn't provide the full or accurate details of your US refusal in your original application and this was part of the reason why your application was refused.

Might be, however there was no place in those forms or anywhere asked me to put your US visa refusal reason?

There is one question asking if you ever get refused applying for a visa anywhere in the world. Failure to show your previous rejections is a serious offense and usually result a 5-years ban. So you are lucky that your application is only refused and there is no ban in progress.
You are the person who take full responsibility for providing information in your study permit application. There were so many people letting their representative do their job without a single knowledge about the possible consequences behind that action.

So next time, take your time and go through every single information and question in every form and document which include in your application.

It means you either lied or intentionally withheld information regarding your background.
