What forms should we upload for upfront medical exams done via eMedical?

What i did was i sent it to them by mail and let the doc to to upload too . i just wanted to be on the save side now it says my medical is approved hope it helps
Ok, Thanks. How long does it take to have a message from CIC confirming the reception of MED exam

My wife and I took our upfront medical exams several weeks ago. Now the "Document checklist" on the PR application requests the following:

"You require a medical exam. Learn how to get an upfront medical exam. In order to submit your online application, you will need to upload the information printout sheet or the IMM 1017B Upfront Medical Report form. Your doctor will give you one of these forms when you complete your medical exam.

If you cannot get a medical exam before the deadline to submit your work permit application, you may submit proof that you have a scheduled an appointment."

I called the doctor's secretary and he said that they don't give 1017B forms anymore because it's all done online.

The only thing they had given us was an "eMedical Tracking sheet" which contains our basic info, photo, and a UMI number. However, all exams (Syphilis Test, Medical Examination, Chest X-ray, HIV test) show as "Referred" or "Required" because the sheet was printed out when we first registered at the doctor's office.

What should we upload?

From where to find the list of hospitals for medical? What all tests are to be done? Where can I find these details?

I have few more concerns- on my checklist page, for PCC it mentions multiple document required. While I have stayed only in India and a week in Thailand (as a tourist). But in India I have spent more than 6 months in 2 cities. So is this multiple a generic thing for whomsoever it applies (for multiple countries ) or does it apply for multiple cities as well in same country?

Yes, the word "multiple" will appear to each and every single applicant even if the applicant does not need to submit a single PCC. If you only need one from India then that is all that is required from you.

And what about reference letter? Like if my spouse is going to accompany me, is his reference letter also required?
1. Also it mentions that details of immediate supervisor and such details to be mentioned (but in this way I can't hide from my manager). Any way out for this?
2. Further, it says that we need to mention all roles and duties for each year at each designation in reference letter. Like I have 3 designation and 5.5/ years - so I will need to include all details at each designation right?
But in this case, my reference letteray go 2-3pages . is that fine?

If you mentioned his work experience in the work history section then placeholders will be created in the document checklist for you to upload reference letter. If you are not claiming points for his work experience (i.e - if all his work experience is foreign) then remove it from the work history and add it only in the personal history. No proof of employment will be required and before you ask this is not misrepresentation. The dependents' foreign work experience is completely irrelevant.

1 - Regarding your work experience it all depends as to whether you claimed points for your current work experience or not. I do not know the specifics so I cannot comment.
2 - Yes, if you have different NOC's even if you worked at the same company then you should have duties/responsibilities for each NOC. Whether you get all of these in a single reference letter or several is all the same.

By foreign experience you mean that experience outside Canada right? And for spouse, experience outside Canada gives no points - so we can exclude that?
But for primary applicant, experience outside Canada does adds points so for this we will need reference letter I guess?

Further, NOCs are same (2173- software engineer). It is just that we are at different roles -like software engineer, senior software engineer, associate consultant, consultant and senior consultant like that. So we need to include all these roles and respectively duties against all these roles?

Wanted to share my upfront medical experience here in Bahrain. I think almost everywhere you get your information sheet on the same day as your exam. Much to my chagrin I found out that here they give the information sheet only after all the results are available and uploaded to the e-medical website. What this means is that you will have to wait for a good 7-10 days to get this information sheet and hence your e-APR is delayed by that much time. In the overall process, this seems like a fairly small delay but if for instance your IELTS score was expiring within a few days of your medicals, you'd be in trouble. Ideally, I should have done my medicals a week before the draw as I was quite sure of getting an ITA. Even here, the hospital misguided and said I can't do it without an ITA. Turns out, we can!

Hello,

My husband has a NON-DEPENDENT 12yr old child, living with the mother in a different country. Not included as dependent on our express entry online application but included in the "family members" section. The question is DOES HE ALSO HAVE TO DO A MEDICAL? I am the principal applicant
