if you want them to contact CRA for your income information you can sign it or if you choose to provide it then its just what is more convenient for you. yu understan?

Even after one more X-Ray (No sign of TB as per doctor) yesterday, they still want to do a CT-Scan next month to rule out TB. Three Sputum samples were given for the second time. They said the Sputum test report will come after 7 weeks.

Same situation. Asking to do medical twice especially X-ray. Where was medical done? Is it Bangalore.

You have to fill out form on computer. Answer yes in question 8 and 16 (if you have a co-signer). Print form and manually sign it. Scan form and upload it.
