What is identification card in US or India ? Please suggest

I need to fill details for identification card for application to CIO. I am from India currently in US on visa.

I have from India - Driver license, PAN card no.

From US I have only ITIN no. and my husband has Social Security.

What is the appropriate ID card to be filled ?

I put the election ID card number there, which is unique to each Indian citizen. But I don't think they're much bothered about that anyway, so if you don't have it, just put N/A.
