I got schedule 4 request today in the morning but I am not sure where and how to upload it??
My profile is locked and there is no place I see to upload it.

Please share how to upload it. All inputs will be appreciated.

Click on view submitted application and then continue...U ll see an additional slot being added for schedule 4 forms
