Urgent please help- Additional family information form

For PGP paper application , does the additional family information form have to have original signatures ? Can it be a scanned copy. I have looked through the guide and only the Schedule A background declaration was mentioned specifically to have original signatures.
The form with the original signatures that I have has my mom's parents name interchanged. That is her dad info is listed under mother and vice versa. Both of them are deceased.

I am thinking of asking her to correct the form, sign and scan it back to me so I can send in the application. The urgency is that my brother who is a dependent on her application turn 22 in a few days time.


Please help
 
All my forms had original signature. How are you applying?
 
Then they have to have originally signatures. Print them and sign them.
