Yes dear, I'm waiting to hear from the MPs office latest by Monday and then I will draft a letter to explain. I'm sure they will show mercy because it wasn't intentional. In the meantime me and my wife are gathering all our ongoing communication from the time we applied until date. So by next week we will send all our evidence to AVO.Phewwww... then thats good does not show you tried to conceal DOB so you may have to explain the change of name ....
Thank you for sharing your experience on here. Finally my application has been approved!! .. Many thanks to everyone on this forum for their kind and great support in every aspect of my application, I couldn't have done it myself and most especially I'm grateful to God for making this possible coz at some point all hope was lost. I was drained and couldn't think straight coz the mistake I made on schedule A background/declaration form IMM 5669 would have been seen and read as misrepresentation and I'm glad I acted fast by following my guts even though I had some friends and family members telling me to let it slide. My advise to other members on this forum is to be straight with immigration and try as much as possible not to have anything under your sleeve as they are looking for one minor mistake to capitalize on your application if not properly addressed. I've since believe that when you stay ahead of them though it might slow the whole processing down but you will definitely come out victorious, special thanks to Cutie101 for standing by me in every process and believe me,she did more than what my representative did,she followed up every process as if I was her client and she nvr stopped encouraging me ... I want to also thank Ugoswife, Lagosbabe and bbcanada for their immensely support and encouragement from the day I realize the mistake and also every other member that supported me "God is indeed the Greatest"!!
Thank you for sharing your experience on here.
I did sent an updated IMM 5669 by correcting the error on question 4d last week but forgot to update my Shedule A background/declaration question 6. After reading your post, I then review my application but realized that I did not update Shedule A IMM 5669 as they both appears to have similar form number.
How do I attack this situation or do have have to write another cover letter for this too?
Anyone with better idea can pm me or advice with sample of persuasive cover letter to address this issue.
Thank you all in advance.
I think you need to also address question 6 as IMM 5669 is also the same form as Schedule A, So what you need to do is write them and let them know it was a mistake and prior to the time you were correcting the question 4 you didn't notice the 6 ... They just need you to acknowledge the mistake and correct it before they make decision.Thank you for sharing your experience on here.

We therefore urge you to read and follow carefully the instructions outlined in this letter without delay.
PHOTOGRAPHS
You must also provide two passport photos for every person receiving a visa on your application. Photograph specifications can be found at: www.cic.gc.ca/english/information/applications/guides/pdf/5445EB-e.pdf. Print the specifications and bring them with you to a commercial photographer. A visa will not be issued if your photo does not match the specifications listed in the link above.
**** PLEASE NOTE:
Failure to provide the required travel document(s) within 15 days from the date of this letter will lead us to conclude that you are no longer interested in immigrating to Canada and your application will be refused based on the information that is available in your file. Once the application is refused, you would be able to reapply, but you would be required to submit a new application and pay a new processing fee. The new application would be assessed according to the Canadian Immigration legislations in force at the time that the new application was filed.
Please note that we will strive to meet the foregoing service standards. However, there may be occasions when we cannot issue your visas within these standards. Accordingly, you are advised not to commit to any travel plans, purchase tickets, or take any irrevocable action such as selling property or resign employment until you have received your immigrant visas.
Kindly be informed that we will not respond to status update queries once you have submitted your passports. We ask for your patience while we prepare your visas.
THE FOLLOWING INFORMATION AND DOCUMENTATION WILL BE REQUIRED TO ISSUE YOUR VISA:
Ø Medical: Complete or provide proof of completion of the Immigration medical examination
Ø Passport/Travel Document: Valid passport submitted after medicals are completed
☐ UPDATED FAMILY INFORMATION: You must advise us of any change in your marital status or family composition. All family members must be listed on your IMM008 and Additional Family Information forms. Failure to comply may result in a delay in the issuance of your visa.
☐ PASSPORTS: Ensure you read and follow the points below:
☐ TWO (2) PHOTOS FOR EACH FAMILY MEMBER:
- There is an individual passport for each family member accompanying you to Canada. Children must have their own passports.
- Each passport is valid for at least 6 months from the time you come for the visa.
- Each passport has sufficient free pages (two required for our visa, plus pages for any transit visa)
These photographs must follow the photo specifications outlined in the following link: www.cic.gc.ca/english/information/applications/guides/pdf/5445EB-e.pdf. Ensure they are taken with a WHITE BACKGROUND and that the SIZE OF THE FACE is between 31 – 36 mm. Photos should not be stapled, glued or pinned (you can paperclip or attach them using an envelope). The photographs must be unblemished as they will be used to produce your Permanent Resident Card once you are in Canada.

Sigh. Clearly IOM is a rip off. I paid about 42,000 for my medicals. Na wa.Congrats to you... Finally the wait is over. Q-life in VI is good and it cost N28,000, you have to book ahead tho but i am sure it is just a day interval. Status get updated within 2 working days.
Congrats! As for doing your medicals while pregnant, some (not all) doctors advice against it (cos of the x-ray) but I did a lot of research online and there's nothing that has proven the x-ray to be harmful enough for an unborn child. The radiation is very low plus it's a chest x-ray not an abdominal one. It's all personal preference though. You can choose to hold on till your baby is born (your application will be put on hold till that time AND you'll have to add your baby your application after birth cos family configuration has changed... I think you pay $150 extra when updating with AVO) OR you can choose to go ahead with having the X-ray done. You'll be asked to sign an undertaking at the hospital if you choose to go ahead. Just make sure you are given a double-coated lead apron to cover your tummy (this is done just as a preventive measure incase any radiation bounces off). I hope this helpsLadies and Gentlemen in the house, I just got MR and PPR just as predicted buy BBC! All glory to God.
