I have not received my PR yet and waiting in the pool.

I am eligible for medical EI (you can get EI with just work permit)

https://www.canada.ca/en/employment-social-development/programs/ei/ei-list/reports/sickness.html

But when I get invited for EE CEC and write LOE (Letter of explanation for the period I was in Canada)
would I have to mention in the letter I received (or receiving) medical EI ? Would this somehow effect PR?
(there is also such thing as inadmissibility to Canada if you spend too much health care)

Thank you for your help.

Getting EI (medical or otherwise) will not affect your PR application. EI is insurance. You have paid the premiums and therefore you are entitled to get it.

No, you do not need to mention this in the LoE. Yes, there is a medical inadmissibility, but that will be covered by your medical test - your having taken EI will not determine this.

If your medical shows that you'll need more than ~$20,000 a year in medical costs, your application will be denied.
