Is it okay to write "PRESENT" in the column "to"?

Example: Addresses in the last 4 years
(January 2013 to Present)

It is very subjective. What I did was: I wrote the date I signed the papers in the 'To' Column and made a 'remark' with a note saying: Present.

Where did you write the remark? Just beside the date?
 
Yes, just underneath the 'To' date. Make sure you qualify for the Citizenship at the date of signing the application. Had a fancy little arrow from the 'To' date and wrote 'Present'.
 
I didn't Write any N/A in the application and left the space empty. I have no kids for instance and left all this empty. do you guys think they will return the application to me?

The date FROM has to be exactly 4 years prior to the date of your application?

For example: I am applying September 1st 2014, does the FROM field has to be September 1 2010 even if I am living at this place for the last 10 years since 2004?
Or do I write 2004 in the FROM field?

Yes. 4 years only from your date of application.
So you don't need to put your each and every history of 10 years in your application.
I recommend you use Residence calculator.
