My passport is registered in Kerala,India.

From where should I request PCC?From the office of Superintenent of Police or from the passport office??

It doesn't matter where your passport is issued from. You need to get the PCC from each place in the world where u've stayed more than 6 months.
As per india is concern, You have to get it from SP or Commissioner's office. Somtimes area police head offices also issues PCC. If you are staying outside India and u r unable to get PCC directly from india, then you can get PCC from Indian Embassy in the country of your current residence.

Do we need PCC from all the countries we lived since our 18th birthday??

We need to get PCC only if we lived in that country for 6 months or more..Am I correct??
