I would appreciate your help with this one.
I have a question regarding the IMM5409 form (Staturory Declaration of Common-Law Union)
1. Is it required that I submit this forma along with my Open Work Permit application? and
2. Do both signatories (Declarant & Partner) have to be present when signing it with an administrator (notary, commissioner etc)?
