I am currently in Canada doing my Ph.D. and my husband and son will apply for Open Work Permit and Student Permit from outside of Canada.
I have a couple of questions regarding the application forms:
Regarding the Additional Family Information Form 5406 my husband will fill for himself (two versions English and native)
Will I have to fill in an Additional Family Information Form?
I am not applying so....?
The Use of a Family Member Representative for Online Application IMM 5713.
Who fills this form? My husband or I?
In proof of relationship do we need to fill in the Statutory Declaration of a common-low union?
Currently, we have ID's, Marriage Certificate, joint bank statements, birth certificate of our son!
The Statutory Declaration Form requires both parties to be present since it needs to be notarized and I am not there, so?
