Minor personal identification doc

Hi - For minor citizenship application, do we need to keep two personal identification documents apart from Passport?
or we can keep one passport and health insurance card is fine?

Please advise.

Yes.
Based on the document checklist we need to provide following documents:
1. Photocopies of the biographical pages of passport
2. Photocopy of two piece of identification - one of which must have a photo in it - My question here is apart from passport do I need to provide 2 piece of identification and one should have a photo? - Correct.

You need to provide two identification, one of them must have a photo on it. So you can add another copy of the passport bio page and health card or school record or one of the listed docs.

Make sure if you decide to use the passport as a piece of id, to include two copies of passports bio page.
