Hi Guys when you do medicals inland what kind of proof do you send to immigrations ? Is it the receipt they give you at the doctor's or a photocopy of form IMM1017?
 
They will give you a copy of IMM1017 your photo stapled to it, one also goes with your medical folder and another with your x-ray.
 
they photocopied it for me. They said the original is for the doctor to send to cic. CIC call center says they should have given me the original :o
 
This is what I found in the Immigration Guide.Note: You will need to include in your application proof (Copy 2 of Medical Report: Section A, provided by the panel physician) that you completed the medical examination

It does not say photocopy.... not sure what to do since the Doctor said they can't give me the original
 
I had my medical done in the states but from looking online it is the same form used the IMM 1017. I took the printed copy of the IMM1017 with me and was told they did not require it that had they had form (was triplicate carbon copy) then she took my photos from me and stapled one on each page once the exam was finished she tore off the middle copy and gave it to me instructing me that it was to be included with my app the other 2 would go on my exam folder and x-ray folder.
