So I'm writing a letter to the Immigration officer because my brother in law was recently diagnosed with hepatitis and is requested to remove his name under IMM 5406 I just need a little help in how to 'formalize' it.I NEED HELP PLEASE!thanks very much in advance...
 
Why would your brother in law be on your IMM 5406? And why would you remove him?

If you wish to amend the form, fill in a new form with the correct information and send it to the VO, along with a letter explaining the reason for your change.
 
hi thanks for your prompt reply..he did not pass the med exam he was recently diagnosed with HEPATITIS.· A separate and newly completed Additional Family Information form (IMM5406) bearing original signatures for the person(s) you are sponsoring. This must be received at this office by: 12/25/2012
· An original Medical Report Form (IMM1017 Copy 2) signed by a Designated Medical Practitioner (DMP) for xxxxxxxx. This must be received at this office by: 12/25/2012
· A separate and newly completed Additional Family Information form (IMM5406) bearing original signatures for xxxxxxx .This must be received at this office by: 12/25/2012
· A separate and newly completed Schedule A Background Declaration form (IMM 5669) bearing an original signature for xxxxxxx. This must be received at this office by: 12/25/2012
· Passport size photographs (photographs must conform to the specifications outlined in the Guide to Immigrating IMM 3999 and Appendix B Photo Specifications) for xxxxxxx .This must be received at this office by: 12/25/2012
 
my husband put his brother's name in under IMM 5406 the note above was requested by ottawa-pilot immigration...i just dont understand as he is my bro in law not dependant.

I think there's something missing here in my understanding. I get that he's been diagnosed with Hepatitis, though having hepatitis is itself not grounds for refusing someone a PR visa.

Who is the sponsor? Who is the primary applicant? Is your brother-in-law being sponsored? If not, whose IMM 5406 are you trying to revise?

The text you are describing sounds like you are adding someone to the application - you wouldn't need a medical report for someone who has already been examined.

So, without understanding your situation, if you need to revise documents at the direction of the visa office (which it sounds like you are doing) then I'd just include a copy of the VO's letter and the revised documents.

Right. That makes absolutely no sense. It sounds like there is a point of confusion here.

I assume you are sponsoring your husband. I'm not sure why you're brother-in-law's hepatitis matters here, but spouses and dependents are excessive demand exempt in any case, though they may require treatment.

Otherwise, it sounds like the VO has made a mistake, in which case you need to be explaining that to them.

Compugeek thanks for your time reading.... please share or help me how to make a letter to the officer explaining.i am really confused my brother in law went for medical and i sent money too its useless.we all just decided to remove his name under IMM0008 i guess my husband made a mistake on PAGE 3 of 6 "dependants" under personal details he added his brothers name.

Ah! Now it makes sense. The VO thinks your brother-in-law is your spouse's dependent.

You need to correct your spouse/partner's IMM 0008 and remove the brother-in-law - that's really what the VO needs.

Then return that to the VO. Include a cover letter and explain that the error was not omission of documents, but rather inclusion of your spouse/partner's brother-in-law on the IMM 0008 and that you have included a corrected IMM 0008. You might want to include a copy of your brother-in-law's birth certificate to prove that he is a sibling and not a dependent.
 
thanks computergeek...thank you so much for you excellent explanation....i asked my husband to get power of attorney and explain about the mistake.
