How to fill IMM 5476e form for appointing representative

Hi, I’m trying to appoint myself as representative for my parents visitor visa application and they are asking to fill Imm 5476e form. While doing so, IRCC has rejected twice saying we need hand signatures - which we did. We put our signatures and had them scanned. Still not sure why they’re returning back the form. I’m afraid to submit it for 3rd time. Anyone with any thoughts about this?
