Depends on the province but the person who requires medical care in the home would provide his/her assessment from their local health authority. This would describe the in-home care required due to medical needs. Also doctor/specialist, OT assessments on the in-home requirements and care needed.
Through the local health authority, some in-home care is covered by provincial health care so would need to describe what is needed above and beyond "free" care.
