I agree. I've seen some cases on this forum where the VO sent a letter requiring explanation or proof for something or a rejection will be issued. I think there have also been a case or two where they outright sent a rejection email.

Yes, but depends on the nature of the basis of rejection. Usually, ineligible work experience or falling short of 12 months can get you rejected right away.
