Which form? Usually you would print out an additional page, and attach upload it in the section for optional documents (e.g. where it says you can upload the letter of explanation. If u have a letter of explanation, do a single pdf that includes the LOE + additional page, and quote in the LOE that you're attaching the additional page and any other document that u wish to upload).

And do i need to do anything else?

My file is currently managed through lawyer representative, but they are not the fastest moving so i'd like to handle the upfront medical exam myself.
