IMM5406: Confusion about "Signatures"

Hello All,

I recently received an email from CIC requesting IMM5406. The email states following:

Additional Family Info IMM5406: A separate and newly completed Additional Family Information form (IMM5
406) bearing original signatures.
This must be received at this office by 2017/10/24

I am confused about who needs to fill this form and whose signatures I need to complete it. I am applying under CEC and I am not married, nor sponsoring my family at this moment.

I did some research and got even more confused as everybody has a different case.

It would be great if anyone of you can help me complete this form appropriately.

I have a similar confusion. It asks that all family members sign the form but there is only one place for a signature (at the bottom). My husband is the Principal and Only Applicant, and I, his Spouse, am a Canadian Citizen sponsoring him. We printed out the form a second time to include all family members' name in their native language and then a third time to include an additional sibling (not enough space on the form) and both her english and Hindi names. It sounds like a LOT of people get a request for additional signatures/forms, and on the signature page of the Document Chacklist it says everyone needs to sign it. Again, is that just "everyone" that is applying for PR, or Everyone mentioned on the form including siblings who live far away? Why does it have to be so unclear!?!
