Pre-Removal Risk Assessment PDF form

Hi everyone i can not find IMM5508 Pre-Removal Risk Assessment form online. Can someone share with me this document as PDF please?

I had interview with Cbsa and officer gave me paper file and he said that the pdf file is also available online. It says on cic website this form is not available for public. I asked to officer can fill it up on computer and sign it he said yeah that's fine so the problem is i can't find this form anywhere and my handwriting is not good... I only have 15 days to submit prra forms....

The form is not available online. The officer made a mistake.

what if scan the file as pdf and fill up like that on the computer and print and sign. is it okay? i do not think they specifically want handwritten but i dont know....
