PR refusals - do they happen at last stages?

So I went thru threads on refusals, read comments on myimmitracker and it seems like in most cases refusals / refections happen during the first checks: PCC issued before one left country, documents missing, wrong NOC codes, wrong format of reference letters - IP1/R10 stages, it seems.
But I also noticed very few cases where one was denied after 5 months, just before expected PPR. What might be the reasons of such a late refusal? When do they actually check job reference letters / duties aka work experience? In the very first stages then? I thought it's an Eligibility check but sounds like Eligibility lands as one of the last sometimes?

Yes apparently they may still reject you 6 months in, for inadequate reference letters, from all that I've read.. you're only in the clear once eligibility is passed, background check and other parameters aside

Personally, I think it's a terrible system overall. What's the point of asking for biometrics or passing medicals before even assessing the eligibility first! Makes zero sense to me

Biometrics are shared with partner countries(M5) for potential risk assessment of incoming immigrants, I'm sure you'd rather this process start before everything else is completed and not delay the overall processing time by another month.

Gotcha. Well that makes sense.. But within their internal workloads, no need for anything else (medical, background, etc.) to take precedence over something as fundamental as eligibility..

An application can be rejected at any time, even after 7-10 months. My Eligibility check didn’t even start until I hit 6 months. After Eligibility is cleared, there is a very low chance for rejection but it’s still a possibility
