hi all! I've noticed that Quebec has quietly updated the payment form A0591A0 in the past month or so. it is different than the forms I downloaded in January, so for those who have saved their files locally should make sure that they are up to date. I've copied & pasted the information & link below.
If you submit the old form after April 1, you'll be rejected. A notice of the update is not listed on the pages with the CSQ application kits. You can use the new form immediately.
Payment by credit card
For any application to be submitted in Québec
If you wish to pay by credit card, enclose with your application the 2018-09 version of the Payment by credit card form (Dynamic PDF, 538 Kb). Only this version will be accepted as of April 1, 2019. After this date, any application sent using the previous version of the form (2016-08) will not be processed and will be automatically returned to you.
