IMM 5475 need help to fill

I too got this request from CIC to submit the 5475 form and I'm not ab;e to get in touch with CIC call center person. I choosed address change option and its keeps on waiting.

Also if Location of office is Sydney, nova Scotia, what will the Name of the Office you have submitted the application : ????

Can someone help me with IMM5475 Form, i submitted the form and they asked for replacement. What I am doing wrong? What is the Office location and office name? I live in USA and submitted online. My wife is primary person for application and i am secondary applicant. Please help.
