FSW Application form Query - Additional Dependent Form

Hi

In FSW application form , there is one form in which we need to fill up details for dependents , It is ADDITIONAL DEPENDENTS FORM. In this form , which family;s name we need to fill with it.?
My wife and i go to canada . will i need to fill my mother and father name with it ?

Dependents only include your spouse and children. Parents do not fall under the definition of "dependents" as per CIC.

The Generic application form can accommodate upto 5 dependents. The Additional Dependents form is used to provide details of your 6th dependent n onward.

I know now that In form IMM0008 Generic Application Form, we need to fill details for our Spouse and Childrens only.

But in The Additional Dependents form 5604, for which family members details need to fill in it.? i have mother and father in my family . and i dont have any childrens. So in this form i need to fill my parents details ? because i have read on website somewhere that if you dont include yoy family members details in form then you will not sponser them on later date.

please guide me about this.

U don't need to fill additional dependant form for parents

Parents will be included under Additional family info. When u want to sponsor them,the data will be captured from this form for parents

Perhaps you are confusing between:
1. IMM5604 Form which is a "DECLARATION FROM NON-ACCOMPANYING PARENT/GUARDIAN FOR MINORS IMMIGRATING TO CANADA" hence not applicable to you.
2. IMM0008DEP Form titled "Additional Dependents/Declaration Form" (This form is used for including the 6th dependent and onward if applicable as i mentioned above)
3. IMM5406 Form titled "Additional Family Information"

It is the IMM5406 Form ("Additional Family Information") where you need to enter your parents and siblings details so that they may be considered for sponsorship in future.

2. IMM0008DEP Form titled "Additional Dependents/Declaration Form" (This form is used for including the 6th dependent and onward if applicable as i mentioned above)
3. IMM5406 Form titled "Additional Family Information"

It is the IMM5406 Form ("Additional Family Information") where you need to enter your parents and siblings details so that they may be considered for sponsorship in future.

That's right. No need to include the Additional Dependents Form in you application package.

hi
can anyone authentically and surely confirm about the forms to fill for fsw 2014? I want to apply for myself along with my wife and 3 children. Before spouse individually had to fill a similar kind of form (as Generic Application Form) but now there is not an individual form as I mentioned but only one form 'Generic Application Form' accommodates spouse and dependants accompanying.
Can anyone surely confirm this please
