Wrong birthdate in Medical Form. What to do? Pls help

In medical form, my daughters date of birth is wrong. What to do?
May I required to email to CHC? If yes, pls give email address.

Hi Johnny I would suggest you send a specific case enquiry email to CHC ,but I would like to ask did you filled the correct dat of birth or is the medical hospital or CHC?

hi,

if the D.O.B of your daughter is wrong in the medical  form, that means the CIC has wrong input of that data  in their system. In my opinion you have to co-ordinate with the visa processing office and get it corrected immediately.

well, Medical is done by physician. My one friend suggest that write this issue of DOB with passports u sent to CHC. Is it ok?

Have the DMP correct the birth date on the form.  That is all that is required.
