I have a work permit that expires dec2010.  I returned to the US Oct 15th and I'm to return Nov 27 to work a few more weeks requested by my Canadian employer.  I need to know if I can apply for an extention since I am currently in the US or if I need to reapply for a work permit and if so does the Canadian employer need to do another LMO.

my second question is regarding a SIN.  Do I send the original work permit with the application?  and what other documentation is needed?

Yes your employer needs to reapply for a new LMO. Under new rules, you cannot extend a work visa unless you have the positive LMO.

Your SIN is only valid for the length of time your work visa states. Once your visa expires, so will your SIN. To get a new SIN, you need the LMO and new work visa.
